Common questions on the acne clinic

Does the questionnaire replace the medical examination?
No, never. The form collects the history — diagnosis, severity grading and treatment decisions are made exclusively by the doctor in the appointment. It makes the appointment better, not redundant.
May minors complete the form themselves?
Consent to data processing for minors depends on their capacity to understand, or comes via guardians — just like with your paper form. In practice: the link goes to the parent e-mail from the booking, and teenagers and parents can complete it together. Align the specifics with your data protection officer.
How secure is the patients' information?
Skin details are health data under Art. 9 GDPR — and we treat them accordingly: hosting exclusively in Germany, encrypted transfer, tenant-isolated storage per practice and an Art. 28 GDPR DPA. You collect explicit consent as the first step of the form.
Why not just Google Forms?
Google Forms stores data with a US corporation — a risk no practice wants for Art. 9 GDPR health data, and you will not get a healthcare-ready DPA there. It also lacks real multi-level branching logic, photo upload with virus scanning and a front end with your practice branding instead of Google styling.
Do teenagers really answer such a form honestly?
In our experience, more honestly than in the consulting room: alone, without time pressure, without a parent next to them and on their familiar phone, the threshold drops — especially for cycle, diet or mental-health questions. Make the sensitive questions optional so there is no pressure.
Can we use the form for follow-up appointments too?
