For occupational therapy practices

Prescription & history before the first appointment — not during it

Your patients upload their therapy prescription as a photo and complete the intake form calmly in advance. You start the first appointment with assessment, not paperwork.

Create intake form

~15 Min of treatment time gained per first appointment when intake happens digitally in advance

The first appointment in many OT practices

Typing up prescription data

Diagnosis, prescribed quantity, key symptoms, referring practice — it is all on the paper prescription and has to be typed into your records by hand. With illegible handwriting it becomes detective work.

"I forgot the prescription"

The patient turns up without the prescription — and you cannot start treatment without a valid one. The slot is lost or turns into pure admin.

History-taking eats the first session

Pre-existing conditions, everyday limitations, therapy goals — important questions, but they cost half the first session. Little time remains for assessment and first therapeutic steps.

How intake works with Questee

  1. 1

Set up your intake form once

Fields for prescription data (diagnosis, prescribed quantity, referring doctor), history questions and a photo upload for the prescription. With AI assistance the form is ready in 30 seconds — you just refine it.

  2. 2

    Patient completes it at home

You send the link with the appointment confirmation. The patient photographs the prescription, uploads it and answers the questions without waiting-room rush — draft saving included.

  3. 3

You review in advance and treat from minute one

Prescription photo and answers are with you, structured, before the appointment. If something is missing on the prescription you resolve it beforehand — and the first session belongs to therapy.

Built for health data

Prescription photo upload

Patients photograph the prescription form right on their phone — no scanner, no fax.

Hosted in Germany

Art. 9 GDPR health data stays on German servers, DPA included — no US cloud.

Conditional logic

Paediatric questions only for children, hand-therapy questions only for hand therapy — the form adapts.

Save & resume

Anyone who needs to find the prescription first can pause and resume later with the same link.

Email notification

As soon as a patient submits the form your team knows — nothing slips through.

Common questions from OT practices

May I collect health data online at all?
Yes — provided processing is secured in line with GDPR. Health data is a special category under Art. 9 GDPR and needs a legal basis (usually consent or the treatment contract), encrypted transfer and a data processing agreement with the provider. Questee hosts in Germany, transfers data encrypted only and provides the Art. 28 GDPR DPA.
Why not just Google Forms or Typeform?
Both store data with US companies — a privacy risk you are accountable for when diagnoses and medical histories are involved. Google Forms also offers no clean DPA process for therapy practices. Questee was built for exactly this case: hosted in Germany, tenant-isolated storage, downloadable DPA.
Does this replace my practice software or insurance billing?
No, deliberately not. Questee is the structured pre-appointment intake: prescription photo, master data, history. Insurance billing continues through your practice management software — Questee claims no billing integration. You transfer the verified data into your records instead of typing it off paper.
What if a patient has no smartphone?
The form works on any device with a browser — tablet, laptop, PC. Often relatives complete it, especially for older patients or, in paediatrics, the parents. For the few cases without internet the paper form remains a fallback — digital intake takes the bulk off your plate, not every single case.
Which fields belong in an OT intake form?
A proven structure: master data, referring doctor and diagnosis as per prescription, prescribed quantity and remedy, prescription photo upload, previous therapies, everyday limitations in their own words, therapy goals from the patient's view, plus logistics (appointment preferences, home visit yes/no). Conditional logic shows paediatric or neurology blocks only when relevant.
Can other patients see the answers?
No. Each patient only sees their own form; answers are stored tenant-isolated in your Questee account and visible only to you and your team. Answers are never emailed in plain text — the notification only says a new intake has arrived.
How quickly can I build such a form?
With AI generation you describe what you need in one sentence ("OT intake form with prescription data and photo upload") — the draft is ready in about 30 seconds. Then you adjust wording and order to your practice. Realistically you are live in under half an hour.
