For veterinary practices

New-patient intake — before the waiting room fills up

New clients record their animal, pre-existing conditions, vaccination status and previous vet in advance — at home, calmly, with the vaccination record on the table. Reception greets instead of retyping.

~10 Min less reception time per new intake — gold dust with a full waiting room

Recognise this from reception?

Clipboard versus waiting-room stress

The new owner fills in the intake sheet while the dog pulls on the lead, the phone rings and three patients wait. The handwriting matches the situation — and the assistant retypes it all.

The vaccination record is at home

"Vaccinated? I think so, sometime last year." Without the record and previous-vet details, the history starts with gaps — and the practice later phones the old vet to fill them.

Pre-existing conditions surface too late

The cat's thyroid medication only comes up in the consult room — if at all. Asked in a structured way beforehand, it would already be on file.

How intake works with Questee

  1. 1

    Create the intake form once

Owner details, species, breed, age, pre-existing conditions, current medication, vaccination status, previous vet, reason for the visit. The AI drafts it in 30 seconds — the team adjusts it without IT skills.

  2. 2

Send the link with the appointment confirmation

New clients get the form link by e-mail or find it on the practice website. They complete it at home — vaccination record to hand — and upload it as a photo right away.

  3. 3

A complete record before the first visit

The practice sees everything structured in advance: conditions, medication, vaccination photo. Reception greets, the vet starts the consult informed — no clipboard, no retyping.

Built for daily practice life

Vaccination-record upload

Owners photograph the record straight into the form — no "I'll bring it next time".

Conditional logic per species

Dog, cat, rabbit or reptile — the form only asks the questions that fit the species.

Draft saving

If the record needs finding first, owners simply continue later — same link.

E-mail notification

Every new intake lands in the practice inbox instantly — ready to transfer to the file.

Questions from veterinary practices

Does the form replace the vet's history-taking?
No — it prepares it. The form gathers the facts (conditions, medication, vaccination status, previous vet) otherwise laboriously collected at reception. The medical assessment and history conversation naturally stay with the vet — who simply starts with complete information instead of from zero.
Is owner data in an online form GDPR-compliant?
Yes — Questee hosts exclusively in Germany, transfers encrypted, stores tenant-isolated and provides the Art. 28 GDPR data processing agreement. That puts the practice on solid ground; the website's privacy policy simply references the form and the DPA.
Why not keep the PDF form we already have?
The PDF must be printed, filled in, brought along or scanned — in reality it ends up back on a clipboard at reception. The online form enforces complete required fields, accepts the vaccination record as a photo and delivers data legibly and structured — nobody deciphers handwriting or retypes.
What about emergencies arriving without the form?
Emergencies are of course treated immediately — the form is for planned first appointments. Practical bonus: the assistant can run through the intake on a reception tablet together with the owner while they wait. The emergency record then lands complete in the same overview.
Can we ask different questions per species?
Yes — via conditional logic. If the owner picks "dog", questions on breed, neutering and travel abroad follow; for "reptile" it is husbandry and terrarium setup. The form stays short for everyone because irrelevant questions never appear.
How does the data get into our practice software?
Responses export as a spreadsheet or can be forwarded to your own systems via webhook — there is no ready-made integration with specific veterinary practice software. What works in practice: the assistant transfers the structured details in seconds via copy and paste — far faster and less error-prone than retyping handwriting.
Do owners actually complete it in advance?
Completion is high when the link sits right in the appointment confirmation — with a line like "So we can focus on your animal instead of paperwork". One question per screen on the phone lowers the barrier; draft saving absorbs interruptions. Anyone who still skips it completes it on the reception tablet — still better than the clipboard.
