For GP practices

The check-up questionnaire patients complete better at home

Family history, lifestyle, vaccination status — answers guessed in the waiting room but checkable at home. Captured digitally in advance, legible at the practice.

How the check-up runs today

Family history off the top of the head

"Did your father have heart problems?" — in the waiting room it is a guess; at home you could call your mother. Exactly the details that matter for risk assessment are the least reliable.

Paper forms create double work

Print, hand out, decipher, retype, file — the health questionnaire occupies your assistant longer than the patient. And incomplete forms only surface once the patient is already in the consulting room.

Lifestyle questions in passing

Smoking, alcohol, exercise, diet — under time pressure and within earshot of reception, answers come out polished. A quiet minute at home yields more honest information.

The digital route with Questee

  1. 1

    Digitise your screening form once

Your existing health questionnaire becomes an online form: family history, pre-existing conditions, medication, vaccinations, lifestyle block. The AI drafts it from your description in 30 seconds — you review and refine clinically.

  2. 2

    Patients research calmly at home

The link arrives with the appointment confirmation. At home the patient can fetch their vaccination card, ask relatives about family history, and save drafts. Conditional logic shows smoking detail questions only to smokers.

  3. 3

The practice starts with the full picture

Before the appointment the form is complete and legible — required fields prevent gaps. Your assistant retypes nothing, and the consultation spends its time on assessment and advice instead of data collection.

Features for the screening practice

Hosted in Germany + DPA

Health data (Art. 9 GDPR) stays on German servers — Art. 28 DPA included, no US cloud.

Conditional logic

Non-smokers skip the tobacco block, women and men see the screening questions relevant to them — the form stays short.

Save & resume

Find the vaccination card, call the parents, continue later — the form waits under the same link.

One per screen, any device

One question per screen in the browser — phone, tablet or laptop, no app and no account.

Arrival notification

The practice learns by e-mail when a form arrives — incomplete preparation surfaces before the appointment, not during it.

Practice branding (Pro)

Your own logo and colours — the form looks like your practice, not an anonymous tool.

Less than a toner cartridge per quarter

Free to try (3 forms, 100 responses/month). Pro for daily practice: unlimited forms, your branding, AI included — €12/month or €9/month annually.

Questions about the digital screening form

Is an online questionnaire GDPR-compliant for health data?
Yes, given the right framework: health data is a special category under Art. 9 GDPR and requires explicit consent — which you collect as the first step in the form. Questee hosts exclusively in Germany, transfers encrypted and provides the Art. 28 GDPR DPA. That gives you the building blocks your data protection officer wants to see.
What about patients who do not use the internet?
The paper form remains as a fallback — nobody is excluded. In practice, the check-up age groups are happy to complete forms digitally. Even if two thirds answer in advance, your assistant's retyping workload drops noticeably. Alternatively, a tablet at reception can show the same form.
Why not just e-mail the PDF form?
The PDF comes back as a kitchen-table photo, half completed, and answers land unencrypted in the practice inbox — a data protection problem for health data. The online form enforces completeness via required fields, branches intelligently and stores answers structured and encrypted instead of as mail attachments.
Can we adapt the form to our practice?
Completely — every question, help text and order is freely configurable. You can add your own focus areas (e.g. travel history, skin-cancer screening pre-questions, sports medicine blocks) and on the Pro plan apply your practice logo and colours.
How do the answers get into our practice software?
Via export or webhook — Questee deliberately does not offer a direct interface to practice management systems. Most practices open the form in the dashboard and transfer the relevant points into the record. That is still orders of magnitude faster than deciphering and retyping handwriting.
Does the form suit other screening appointments too?
Yes — the same principle works for skin-cancer screening, adolescent check-ups, DMP documentation prep or vaccination status checks. You create a separate form with its own link per occasion; unlimited on the Pro plan.
Does the form answer patients' medical questions?
No — the form collects information; it does not advise or assess. All medical evaluation, recommendations and the check-up conversation itself remain fully with you and your team. Exactly this clear separation of roles makes it safe to use.
