Answers from the practice

May we collect health data via an online form?
Yes, with the right safeguards: health data is a special category under Art. 9 GDPR. You need patient consent, encrypted transfer and a DPA with the provider. Questee hosts in Germany, provides the DPA (Art. 28 GDPR) and uses no US subprocessors. Google Forms does not meet these requirements.
Does this replace the signature on the history form?
The patient enters the data digitally; confirmation happens at the visit — many practices print the completed PDF for signature, or confirm via the signature pad in their practice software. The time gain remains: reviewed, legible data instead of clipboard handwriting.
Do the data get into charly or Dampsoft?
Via PDF or CSV export. Practice software has history modules but no patient frontend for home use — Questee closes exactly that upstream gap: the patient enters, the assistant reviews and transfers. The PDF files directly into the digital patient record.
What about older patients without a smartphone?
The form runs in any browser — on a PC or a relative's tablet too. For everyone else, the clipboard remains as a fallback, or the assistant enters the answers directly during intake. Data stays consistent regardless of the path.
How does prophylaxis recall work concretely?
You create a short recall form (preferred date, weekday, morning/afternoon, callback number) and e-mail the link to due patients. Responses come in structured, the team books in batches — instead of working phone lists between treatments.
How long are responses stored?
You define it per form. Many practices export the history as PDF into the patient record (where the 10-year dental retention duty applies) and let Questee auto-delete the online response afterwards — retention policy with audit trail included.
